OPENING, DOWNLOADING AND SAVING A PDF
To open a PDFs document you need Acrobat Reader, download it for free from the Adobe website.
The forms can be downloaded and either
- printed and completed with a pen, or
- completed on screen and then saved and printed.
To SAVE a form completed on screen, follow these instructions:
- Select the print icon at the top OR, “Right-click” your mouse and Click “Print”
- Choose a destination – e.g. Adobe PDF
- Click “Print”
- Name the file. Use the project street name or the client’s name and the date, e.g., Anywhere St 2016-08-16
- Select the folder on your computer to save the file to
- Click “Save”.

Important information:
- An applicant checklist must be provided with a building consent application form
- Choose the correct type of your building project i.e., dwelling checklist for residential projects
- Designers – please check the checklist is completed in full and that all information requested has been provided
- Missing information causes delays (cannot be accepted for lodgement)

Other documentation required with the application:
- Fill in the application form completely. See the APPLICATION FORM GUIDE.
- Also, complete and provide the relevant Applicant Checklist(s) making sure that you have checked that all requested information has been provided.
- If there is Restricted Building Work, a Certificate of Work is also required. Ensure that the COW is completed in full.
- If there are specified systems (safety systems) indicated on the plans, you must provide details about these systems. Complete and provide the relevant Compliance Schedule Details form(s).
- BEFORE you submit your application documentation, please check it for completeness. MBIE requires that any incomplete applications will be rejected (i.e. incomplete forms/missing documents).
Major Amendment (Form 2)

Important information:
- Any major changes to an existing building consent require approval.
NOTE: Don’t be confused by MBIE’s terminology of Major Variation; Major Amendment is the correct terminology.
A NEW building consent application, plans, and specifications must be submitted and approved before work begins (Building Act 2004, 45,4 (b)). - Provide the new plans and specifications, relevant Applicant Checklist(s), COW (if required), and Compliance Schedule Details forms with your application.
- If the applicant is not the owner or designer, please provide the owner’s AND designer’s written an agreement for changes.

Minor Variation

Important information:
- The Minor Variation form is not being used by Waipa and Waikato District Councils
- See MBIE examples of Minor variations, and Building (Minor Variations) 2009 legislation, and MBIE Guidance to building consent to amendments
- A handwritten and signed letter may suffice – outlining changes on the plans, and a written description of changes
- Best practice to check with your local Council if it is a MINOR change
- Get the designer’s & owner’s written an agreement to the changes.
Certificate of Design Work (COW) (Form 2A)
- Prescribed Form (Form 2A), or
- MBIE form
Important information:
- Applies to ALL Restricted Building Work projects
- Must be supplied by the Licensed Building Practitioner (LBP) designing the work
- Include with any MAJOR amendments – see LBP scheme
- The wording and order of wording must match the Prescribed or MBIE approved form.

LBP Notification

Important information:
- Applies to ALL Restricted Building Work projects
- Best case scenario is to supply all names and details with BC application
- If not, trade LBP’s details must be supplied BEFORE construction begins
- NO inspections will be booked until ALL LBP’s details are supplied
- LBP licensing details are checked by the BCA (allow 1-2 days)
- Also, use this form if the trade LBP’s for your project change during construction. Notification of changes to LBP is required by the Building Act 2004, section 87.

Notice of Owner-Builder (Form 2C)

Statutory Declaration (Form 2B)
- Prescribed form (Form 2B – need to accompany the owner-builder exemption form)
- Prescribed form (Building (Forms) Regulations 2004 – layout)
Important information:
- This is for Owner-Builders carrying out Restricted Building Work on their own home
- Complete BOTH Forms BEFORE construction begins
- Use when a homeowner starts using the Owner-Builder Obligations
- Use when a homeowner stops using the exemption

Record of Work (ROW) (Form 6A)
- MBIE form (Interactive PDF form)
Important information:
- Applies to ALL Restricted Building Work (RBW)
- See Homeowners rights and obligations
- Also called a Memorandum from a LBP (see LBP scheme)
- ALL trade LBP’s must supply the owner, and the Council, with their Record of Work
- Must be supplied to Council BEFORE, or an application for a CCC (Building Act 2004, s88).

Code Compliance Certificate (CCC) (Form 6)

Important information:
- Must apply for this as soon as practicable after completion of a project
- If restricted building work (RBW), the owner must supply ALL Records of Work (row) with the CCC application (Building Act 2004, 92(2a))
- Ideally, completion of work takes place within 2 years of BC application, or an extension of time is required
- Tell council if the ownership, or the legal description, of a property has changed since BC was granted

Certificate of Acceptance (Form 8)

Important information:
- Applies where urgent building work is carried out (to protect lives or prevent serious damage to property)
- Applies where a previous owner has carried out building work without a building consent
- Have similarities to a CCC that verifies that specified building work complies with the building code

Compliance Schedule Amendment (CS) (Form 11)

- Occurs where an amendment to the specified systems within a building is made through:

Certificate for Public Use (CPU) (Form 15)

Important information:
- Certifies that premises affected by building work are safe for members of the public
- Only applies where a building consent has been granted, but CCC has not yet been issued
- Is likely to involve some form of inspection
- An owner of a building is still expected to apply for CCC after building work completion

Before applying for a Building Consent:
- Check with the Planning Department of your local Council if a Resource Consent is needed
- When you apply for a Building Consent, and
- If you will need to pay Development Contributions.
- Plan ahead, and be aware of the expenses that could be incurred.
Waipa District Council
Development Contribution Information Form:
- Complete this form for any non-residential development (i.e. commercial, industrial, retail developments).
- It is important that this information is received at the front end of the process
- Before an application for a Building Consent or a Land Use Consent is lodged, to verify any issues.
Hamilton City Council
Please provide 2 sets of plans and specifications:
- For projects worth $10,000 or more please supply a third-floor plan.
- A Pre-Application meeting prior to lodging the application is also required.
